Cancellation / No Show Policy

Thank you for choosing Empire Eye and Laser Center. When you schedule an appointment at our office, we set aside enough time to provide you with the highest quality care. Should you need to cancel or reschedule an appointment,  please contact our office as soon as possible, and no later than 24 hours prior to your scheduled appointment. If your appointment is on a Monday, please call our office the Friday prior. This gives us time to schedule other patients who may be waiting for an appointment. Please note that arriving late for an appointment may require the appointment to be rescheduled, which will be evaluated on a case-by-case basis.

  • Patients who fail to show for their scheduled provider or consultation staff appointment, or cancel or reschedule within less than 24 hours of their scheduled appointment time, shall be subject to a “No Show” or “Late Cancellation” fee of $50.
  • Patients who fail to show for their scheduled surgical consult, or cancel or reschedule within less than 24 hours of their scheduled appointment time, shall be subject to a “No Show” or “Late Cancellation” fee of $100.
  • Patients who fail to show for their scheduled surgery or in-office procedure, or cancel or reschedule within less than 24 hours of their scheduled appointment time, shall be subject to a “No Show” or “Late Cancellation” fee of $100 – $250.

  • Emergency Appointment Add Fee: $125
  • DMV Form Completion Fee: $35
  • Cash Pay Cataract Consult Scheduling Fee: $50
  • Medical Record Request: $6

WE TAKE THE HEALTH AND SAFETY OF OUR PATIENTS AND STAFF VERY SERIOUSLY, PLEASE READ THE FOLLOWING:

  • Please do not come in if you have any symptoms of respiratory illness (fever, chills, cough, sore throat, muscle aches, malaise, shortness of breath). Please call your primary care physician and seek medical care and guidance if symptoms are severe.
  • Please do not come in if you are experiencing any loss of taste or smell with the absence of runny nose, congestion.
  • If you have had contact with someone with a confirmed case of COVID-19 within the past 14 days, we ask you to please inform us so we can reschedule your appointment for a later date.
