TriZetto Data Security Incident: Information & Resources
Empire Eye and Laser Center is committed to protecting patient privacy. We are sharing this information because a third-party vendor used for certain healthcare insurance eligibility transactions, TriZetto Provider Solutions (TPS), reported a data security incident that may have involved patient records associated with our practice.
Important Notice
Empire Eye and Laser Center’s internal electronic health records and computer systems were NOT breached and remain secure. This incident occurred on the systems of TriZetto Provider Solutions (TPS), a third-party healthcare insurance eligibility and claims clearinghouse.
What Happened?
On October 2, 2025, TriZetto became aware of suspicious activity within a web portal used to access their systems. TriZetto determined that, beginning in November 2024, an unauthorized actor gained access to certain records related to insurance eligibility verification transactions. TriZetto successfully contained the threat and secured its environment on October 2, 2025.
Empire Eye and Laser Center was notified of the potentially affected individuals in December 2025 and has worked diligently to ensure an accurate and transparent notification process for our patients.
What Information May Have Been Involved?
Depending on the individual, the affected data may have included:
- Patient name
- Primary insured name (if different from patient)
- Address
- Date of birth
- Social Security number
- Insurance member numbers (including, in some cases, Medicare beneficiary identifiers)
- Health insurance company information
- Information related to dependents (where applicable)
The following information was NOT involved in this incident:
- Clinical or Medical Records: Your health history, exam notes, and prescriptions remain secure.
- Financial Information: No payment card or bank account information was affected.
What We Are Doing
We take the security of your information very seriously. In response to this third-party incident, we have:
- Validated the Impact: Confirmed the specific patient list provided by TriZetto.
- Coordinated Protective Services: Ensured TriZetto provides identity monitoring at no cost to our patients.
- Proactive Communication: Alerting our patients via email and this resource page ahead of the arrival of physical mailers.
What TriZetto Is Doing
TriZetto is offering free identity protection services through Kroll, including Credit Monitoring, Fraud Consultation, and Identity Theft Restoration. Official notification letters containing a unique activation code are expected to be mailed on or around February 9, 2026, to those who may have been affected.
What You Should Do
While there is no current evidence that patient information has been misused, we recommend the following preventive steps:
- Watch for your letter: If your data may have been affected, you will receive an official mailer from TriZetto/Kroll with more details about the incident, as well as instructions and a code to enroll in free services at enroll.krollmonitoring.com.
- Monitor Statements: Review your health insurance Explanation of Benefits (EOB) for any unfamiliar charges.
- Check your Credit: Order a free credit report at www.AnnualCreditReport.com.
