If you have received a contact lens prescription from us, or if you have previously ordered contact lenses through our office, simply fill out the form below. Dana will contact you promptly to confirm your order and to arrange for pickup or to discuss delivery options. If you prefer, you can call 661-325-3937, and ask for Dana.
If you have never ordered contact lenses through us, or if you are interested in getting fitted for contact lenses, please call our office and set up an eye exam for contact lenses with one of our excellent optometrists.
